Job Description

Who We Are:
Sunrise Supported Living provides person-centered Supported Living Services to adults with developmental disabilities in Sonoma, Napa and Solano Counties. We support individuals in living as independently as possible in their own homes and communities while maintaining choice, dignity, safety, and meaningful community connections. Our team works alongside each individual to provide customized support based on their goals, preferences, strengths, and needs. We are looking for team members who share our commitment to providing compassionate, reliable, and individualized support. About the Position Sunrise Supported Living is seeking a compassionate, dependable, and organized Case Facilitator to join our team. As a Case Facilitator with Sunrise, you will manage a caseload of 4-7 clients receiving Supported Living Services. You will help coordinate services, support staff, monitor client progress, and ensure each individual receives person-centered support that promotes independence, choice, safety, and community involvement. You will work closely with clients, Direct Support Professionals, families, Regional Center Service Coordinators, and other members of each individual's support team.
What You'll Do:
Manage and support a caseload of approximately 4-7 clients Conduct regular client home visits and check-ins Monitor client goals, progress, health, safety, and overall quality of services Coordinate and communicate with Direct Support Professionals Assist with staff support, follow-up, and problem-solving Review daily service notes and documentation for accuracy and completeness Assist with quarterly reports, annual ISPs, IPP meetings, and team meetings Maintain professional communication with Regional Center Service Coordinators and families Monitor staffing, transportation, medical, behavioral, and changing support needs Follow up on client concerns and ensure important tasks and deadlines are completed Report significant incidents and health or safety concerns promptly Help ensure services remain person-centered and focused on client independence and personal goals Case Facilitator Requirements Bachelor's degree preferred Minimum 2 years of experience in the social services industry Experience working with individuals with developmental disabilities preferred Must successfully complete required fingerprint/background clearance Employment is contingent upon successfully passing a pre-employment drug screening Valid driver's license Acceptable DMV driving record Reliable working vehicle Current auto insurance Must be legally eligible to work in the United States Why Sunrise Supported Living?
