Active Employment:

$250 after 3 months, $250 after 6 months, and $500 after one year of employment

REQUIREMENTS OF THE POSITION

: Must have a valid California Drivers License, personal automobile for use on the job, good DMV Record and auto insurance. Pass a criminal clearance, physical, drug screen, and TB Test Train and supervise adults with intellectual and developmental disabilities in a dynamic, community-based day program in our Fairfield program. Provide training at program site, in the community and employment sites. Education /

Experience:

A high school education or equivalent is required. Previous experience working with adults with intellectual and developmental disabilities preferred. AA/BA Degree Salary Bonus.

Duties/Responsibilities:

• Develop and implement Person Centered Individual Service Plans
• Maintain ongoing data collection of participant objectives in their Person Centered Service Plans
• Supervise community integration activities including workcrews and field trips.
• Assist in transportation of participants to and from their homes to program via SDS Transportation Vans.
• Participate in Staff and Person Centered Planning Meetings.
