General Job Summary As a HOPE Team Outreach Specialist at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions regarding reintegration into the community of person who are homeless. Major Duties and Responsibilities Provide outreach services Conduct assessments and complete referrals Assist clients with obtaining necessary documents to obtain housing Negotiate the criminal justice system and advocate for diversion when appropriate Adhere to contractual obligations Serve as acting representative to committees and community planning Complete required trainings and participate in required meetings Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures. Position Qualifications High School Diploma required Bachelor's degree preferred Level II Background clearance
