Position Summary:
Family Advantage Residential Level III service is designed to help children with psychiatric disabilities increase their functioning so that they can be successful and satisfied in the environments of their choice with the least amount of ongoing professional intervention.
QUALIFICATIONS
(A) graduate of a college or university with a Masters degree in a human service field with less than one year of full-time, post-graduate degree accumulated
MH/DD/SA
experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision shall be provided by a qualified professional with the population served until the individual meets one year of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually; or (B) graduate of a college or university with a bachelor's degree in a human service field with less than two years of full-time, post-bachelor's degree accumulated

The parties shall review the plan annually; or (D) registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with less than four years of full-time accumulated experience in

STAFF REQUIREMENTS

LEVEL III RESIDENTIAL
program shall be under the direction of a person who meets the requirements specified for Qualified Professional (QP) status according to 10A
NCAC 27G.0104.
The QP is responsible for the supervision of other program staff which may include Associate Professionals (AP) and Paraprofessionals who meet the requirements according to 10A NCAC 27G.0104 and who have the knowledge, skills, and abilities required by the population and age to be served. A minimum of one staff member on-site to each eight (8) or fewer participants in average daily attendance shall be maintained as specified in licensure requirements (10 A
NCAC 27G. 1200
).
JOB RESPONSIBILITIES
Provide a minimum standard daily service note that describes the consumer's intervention, the time spent performing the intervention, the effectiveness of interventions and the signature of the staff providing the service. Administer medication and medical care when needed. Be aware of consumer's rights and honor those rights. Follow scheduled activities. Participate in in-service training and other conferences related to assigned duties. Responsible for the care and development of consumers. Assist consumers in the development of self-help skills (personal care and hygiene skills such as cleanliness, grooming, dressing, etc.) social skills (etiquette and social relationships), and other activities. Give supportive counseling to increase consumer's self-worth, purpose, and confidence Conduct staff support for consumer-directed and managed activities. Conduct group and individual psychoeducational activities. Ensure the completion of the comprehensive functional assessment for newly admitted consumers within 30 days of admission. Participate in the development of the PCP for newly admitted Consumers as scheduled by the QP. Assist with the completion of PCPs on an annual basis Ensure and monitor the implementation of each consumer's PCP across all settings.
SPECIFIC RESPONSIBILITIES
(CONTINUED) Conduct Adaptive Skill Training in all functional domains - personal care, domestic, Therapeutic Interventions, communication, leisure, problem-solving, etc. Conduct behavioral interventions/level systems, contracts, and structured behavioral programs. Conduct community integration and support activities to facilitate adjustment, maintenance and enhance skills. Model positive reinforcement, redirection, de-escalation, anticipatory, guidance, etc. Provide training to caregivers, service providers and others who have a legitimate role in addressing the needs identified in the service plan (non-agency staff).
