Location:
Co-located in Forsyth and Stokes Counties. Summary Employment Specialist helps individuals find and maintain meaningful employment opportunities. This role includes career coaching, job placement assistance, employer outreach, resume support, interview preparation, and ongoing employment support. Employment Specialist will work with job seekers facing barriers to employment, due to mental health or substance use challenges. Provides person centered care management, referral and linkage to needed resources for recovery. Is assigned to an interdisciplinary team and works in collaboration with the team. Duty or Responsibility Meets with clients to assess whole person care needs and specifically assess employment goals, skills, and experience. Develops individualized care and employment/career plans. Assists clients with resumes, cover letters, and job applications. Conducts mock interview and provides interview coaching. Identify and develop professional relationships with local employers, match qualified candidates with job opportunities, and track job performance and progress. Follow up with employers and clients to ensure successful employment outcomes. Provides care management assessment/reassessment, development of care management plans, referring and linking to needed services, monitoring/follow up with clients and referrals, provide education for health promotion. Demonstrates the ability to document clearly, concisely and accurately all individual and group meetings. Participates in interdisciplinary treatment planning, consultation activities and ensures all involved parties are aware of the plan of care. Demonstrates the ability to conduct skills groups or individual discussions following the guidelines and principles of Recovery and Trauma Informed Care. Provides crisis intervention consultation to all clients and involves crisis services when needed. Demonstrates an understanding of HIPAA and obtains signed releases of information as needed prior to sharing information. Demonstrates the ability to consistently use appropriate professional boundaries with clients, co-workers, and community stakeholders. Strict compliance with all professional standards, Medicaid and Medicare regulations, agency policies and procedures. Other duties assigned by supervisor or other members of management team Responsibilities of the FEP Employment Specialist include, but are not limited to, the following:
Units Billed Minimum Requirement:
4.5 hours of billable hours of service per day.

Strong interpersonal skills, ability to make effective use of time and prioritize work assignments. Ability to communicate effectively with professionals and clients/families. Must have good communication skills and ability to read and write. Must have computer skills sufficient to document in an electronic medical record. Ability and willingness to work as a team member. Ability to make sound decisions in emergency situations.

Experience:
Prefer Bachelor's Degree in Social Work, Counseling or other Human Services field. Other degrees and equivalent education or work experience will be consider. Must have experience level to meet Qualified Professional (QP) status.
