About The Life Link The Life Link is a respected Santa Fe non-profit dedicated to helping hungry, homeless, and displaced individuals and families achieve self-sufficiency. We break cycles of chronic homelessness, mental illness, trauma, and substance use through a comprehensive approach including emergency assistance, housing, employment services, and advanced substance use and mental health treatment. The Opportunity We are seeking a dedicated and consistent employee for street-based outreach. You will be a vital link, empowering individuals experiencing homelessness to gain housing, employment, and a strong social support network. We employ a positive, client-driven, and strength-based approach in all we do. Key Responsibilities (What You'll Do) Provide contract case management services , including assessment, referral, and follow-up, tailored to the unique needs of clients experiencing homelessness. Conduct community outreach to engage unhoused individuals, build trust, and inform them of available health care and substance abuse services. Coordinate client support services to secure safe, affordable housing, ensure access to treatment, identify educational/employment options, and provide life skills training. Assist clients in accessing crucial Federal, State, and local programs (e.g., Medicaid, financial assistance). Advocate on behalf of clients , including accompanying them to appointments. Maintain accurate patient records and submit all required activity reports promptly. Practice safety protocols and de-escalation techniques at all times, ensuring safety during street case management.
What You'll Bring Skills:
Strong background in community outreach, networking, crisis management, and case management.
Knowledge:
Sensitivity to trauma, cultural factors, and challenges affecting persons with severe mental illness, chemical dependency, and co-occurring disorders. Knowledge of Santa Fe/Espanola area resources is a plus.
Minimum Requirements:
High school diploma or GED Must pass a criminal background check. Must pass a driving record background check
