Wellness Navigator SUMMARY This renewable, full-time, 12-month position serves as the primary point of contact for student wellness navigation, basic needs coordination, and community resource referral through the College's Stepped Care Model. The position provides non-clinical intake, navigation, case coordination, follow-up support, and warm referrals for students experiencing barriers related to wellbeing, academics, or persistence. Working within the Wellness Center model, the position connects students to campus resources and external providers, including healthcare, mental health, housing, food security, and other human service agencies. The role supports basic needs initiatives and student success interventions by helping remove barriers to retention and completion. Coordinates day-to-day basic needs operations and student support services, such as the Panther Pantry, Clothing Closet, Recovery Program, and community resource connections, helping students access resources that support wellbeing, recovery, persistence, and academic success. The position also supports assessment efforts, assists with wellness-related compliance processes, and helps identify gaps and opportunities within the College's student support network.

NATURE AND SCOPE

Serves as the initial point of contact for the College's Stepped Care Model, conducting non-clinical intake and student navigation by identifying student support needs, providing information and guidance, and connecting students to appropriate campus and community resources. Refers all symptoms, medical, mental health-, or acuity-based concerns to licensed clinicians or other qualified professionals for clinical assessment and intervention. Coordinates day-to-day operations and basic needs services including the Panther Pantry, Clothing Closet, Recovery Program, and related initiatives, supporting student access to resources related to food security, housing stability, transportation, childcare, financial hardship, and other basic needs supports. Coordinates student immunization compliance processes, including outreach, record review, retrieval of immunization records from the New York State Immunization Information System (NYSIIS) or other authorized registries, documentation, student communication, and coordination with internal and external partners. Refers questions requiring clinical interpretation, including immunization exemptions, titer results, QuantiFERON testing, or other medical determinations, to a licensed healthcare professional. Assists with the development, coordination, and exploration of referral pathways and community partnerships, including healthcare providers, mental health services, housing and human service agencies, and other community organizations in Tompkins and Cortland Counties, supporting warm hand-offs and student access to care. Participate in Stepped Care Model meetings, serve as a contributing member in Wellness Team discussions, and engage in other student support case review processes, collaborating with Academic Affairs, Community Living & Student Support, and other campus partners to support student success, retention, and coordinated intervention strategies. Maintain accurate documentation and records within case management, compliance, and reporting systems, including tracking referrals, service utilization, outcomes, and student support activities. Support assessment, outreach, and educational programming related to wellness, basic needs, recovery support, and student wellbeing, utilizing data to identify service gaps and opportunities for improvement. Maintain confidentiality of student records and information in accordance with Family Education Rights and Privacy Act (FERPA), applicable regulations, and College policies. Coordinate referrals to community health, mental health, recovery, and social service agencies and maintain collaborative relationships with external providers. Perform other related duties as required or assigned.

MINIMUM QUALIFICATIONS

Associate's degree in Human Services, Social Work, Health Services, Chemical Dependency, Community Health, Psychology, Higher Education, Business, or a related field required; and Two (2) years of experience in student services, healthcare support, human services, social services, case management, care coordination, higher education, or a related field required. Experience working with diverse populations and coordinating referrals and support services required.

Bachelor's degree in Human Services, Social Work, Community Health, Higher Education, or a related field. Experience working in higher education, student affairs, health services, or behavioral health settings. Experience with student support, case management, care coordination, or basic needs programming. Experience supporting food insecurity initiatives, recovery programs, or community health partnerships. Experience using electronic recordkeeping, case management, or compliance tracking systems. Experience supporting immunization compliance, healthcare navigation, or student records management. Experience working with college students, including adult learners and underserved populations. Experience supporting students in recovery, substance use prevention initiatives, or behavioral health programming.
