Performs diversified clerical duties supporting department case management activities. Coordinates patient placement with post-acute care facilities and/or services. Job Responsibility Supports case managers and social workers on assigned units with administrative aspects of transition planning activities. Gathers and processes required documentation to facilitate placement. Communicates with post-acute care facilities to initiate patient transfers. Obtains appropriate insurance authorization for post-acute care services. Arranges for patient transportation upon hospital discharge, as required. May attend interdisciplinary patient rounds.
