Flexible work hours based on participant needs (up to 40 hours/week) As a Case Manager (Employment Specialist), you'll help individuals and families move toward long-term self-sufficiency by connecting them with employment opportunities, career pathways, training resources, and the support services needed to succeed. You'll work directly with participants to identify strengths, overcome barriers, develop career goals, and secure meaningful employment while coordinating housing stabilization and wraparound supports. In this role, you will:

The employee may often or occasionally lift or move durable medical equipment, supplies, or groceries up to 50 pounds. The employee may often or occasionally aid participants with balance (i.e. provide an arm to keep steady) and aid with transferring to and from a wheelchair and/or vehicle. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.
The ability to walk up and down flights of stairs is required. Work Environment The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually quiet or moderate; however, loud noise may be encountered. Qualifications:
Education, Experience,and Certification(s)Required Bachelor's degree preferred. Associate degree in Psychology, Human Services, or a related field with 1 year of relevant experience serving vulnerable populations required OR high school diploma/GED with 3+ years of human services experience required. Minimum 1 year of supported employment experience, including job development, job coaching, vocational support, or workforce services preferred. Experience building partnerships with employers and/or postsecondary or training providers; sales, marketing, or employer outreach experience preferred. Knowledge of case management practices, documentation requirements, and service coordination. Demonstrated ability to conduct structured interviews, complete assessments, identify immediate needs, and document findings accurately. Annual criminal background checks, child abuse clearances, and other required screenings in compliance with ServiceSource, state, and funding agency regulations. Availability and willingness to travel weekly within the local area to perform the job. Availability and willingness to work flexible hours, including evenings and/or weekends, when needed. Valid driver's license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR). Knowledge, Skills,and Abilities Proficiency in Microsoft Office products. Strong verbal and written communication skills. Excellent organizational skills. Detail-oriented and able to carry out work with the highest levels of accuracy. Committed to social inclusion model and strengths-based approach. Knowledge of continuum of care and transition planning. Ability to establish and maintain positive working relationships with participant systems. Knowledge of public benefits (SNAP, TANF, Medicaid, Medicaid Waivers, and Medicare, etc.).
