Certified Supported Living Coach (APD Waiver)

Job Summary Haven Landing™, operated under its parent company Pomelo Properties LLC, is seeking a Certified Supported Living Coach (SLC) to deliver structured, person-centered services to adults with developmental disabilities under Florida's APD iBudget Waiver. This is not a passive caregiving role. It is an independence-driven coaching position focused on skill development, accountability, and Medicaid-compliant service delivery within a regulated environment. You will work directly in community and home settings to support individuals in building sustainable daily living skills, strengthening autonomy, and progressing toward individualized service plan goals. Key Responsibilities Deliver Supported Living services aligned with APD authorizations Coach individuals in daily living skills (budgeting, hygiene, meal planning, home management) Reinforce safety, self-advocacy, and community integration Implement structured skill-building plans based on individualized goals Maintain accurate, timely Medicaid-compliant documentation Track measurable progress and submit service notes in accordance with waiver standards Communicate effectively with families, support coordinators, and interdisciplinary teams Complete required APD trainings and maintain certification compliance Services may include Supported Living, Companion, and Respite based on authorization. Required Qualifications Certified Supported Living Coach (Florida APD) Eligible to provide Medicaid waiver-funded services Level II Background Screening (or ability to complete) Experience supporting individuals with intellectual and developmental disabilities Strong written documentation skills Valid driver's license and reliable transportation

Preferred:
