MAPPS Youth Specialist
Job Summary:
Plans and conducts weekly group patient education sessions and individual education sessions per month with each participant. Maintains a caseload of required number of participants. Makes appropriate referrals to other organizations and agencies. Conducts home visits in order to complete assessments and case plans. Plans and provides cultural, educational and recreational activities for program participants. Serves as a role model for participants and parents, by acting in a manner consistent with the program's and district's philosophy. Completes travel reports and weekly documentation. Documents services provided according to Medicaid specifications.
Qualifications:
Requires a Bachelor's Degree in Social Services or related field and training. Experience and skills for efficient functioning of the position. Equivalent combination of education, training, and experience that provides the required knowledge, skills and abilities.
Working Conditions:
Full time position. This candidate will report to an assigned supervisor. This position will be reviewed at the end of year.
