West Texas Centers is seeking a compassionate, organized, and motivated PASRR/Habilitation Coordinator to join our Local Intellectual and Developmental Disability Authority (LIDDA) team. In this rewarding role, you will advocate for individuals with intellectual and developmental disabilities residing in nursing facilities by coordinating services, developing person-centered plans, and ensuring they have access to the resources and supports needed to achieve their goals.

Conduct monthly face-to-face visits with individuals receiving services. Coordinate and facilitate Service Planning Team (SPT) and Interdisciplinary Team (IDT) meetings. Develop, update, and monitor Habilitation Service Plans and assessments. Coordinate services and referrals to medical, social, educational, vocational, and community resources. Monitor service delivery and individual outcomes to ensure quality and satisfaction. Advocate for individuals' rights, preferences, and independence. Present Community Living Options (CLO) and educate individuals and families about available services. Maintain accurate and timely documentation in compliance with HHSC, TAC, and agency requirements. Complete required authorizations and documentation within established timeframes. Serve as the West Texas Centers PASRR Diversion Coordinator . Coordinate PASRR Evaluations and complete required TMHP data entry. Monitor PASRR alerts and ensure compliance with HHSC performance standards. Train staff regarding PASRR responsibilities and reporting requirements. Travel throughout the service area to meet with individuals and providers. Perform other duties as assigned.

A habilitation coordinator must:

(1) be an employee of a LIDDA; (2) have a bachelor's or advanced degree from an accredited college or university with a major in a social, behavioral, or human service field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice; and (3) have at least one year of experience working directly with individuals with ID or DD.
