Job Description

Position Summary:
The SUDP will perform individual, group, and family sessions. They will conduct chemical dependency assessments and intakes documenting both DSM 5 and ASAM. As well, they will use ASAM for continuing treatment review and discharge. The SUDP will comply with personal, administrative and clinical policies and procedures of the agency. Supervised by the Clinical Supervisor.
Essential Job Functions:
  • Responsible for the case management of each patient.
  • Perform individual, group, and family sessions.
  • Conduct chemical dependency assessments and intakes documenting both DSM 5 and ASAM.
  • Utilize ASAM for continuing treatment review and discharge.
  • Completes in a timely manner, per the Washington Administrative Codes (WAC), all required forms/documentation for each client.
  • Prepares current client progress reports and completion notices and provides them to the courts, probation departments, and referral sources as appropriate.
Knowledge, Education, Experience:
  • Associates degree in health care, social work, psychology or other related field
  • Completed 2,500 hours of supervised experience within the field of substance abuse or other related field
  • Prior experience and in-depth knowledge of SUD assessment
  • Prior experience utilizing motivational interviewing, group facilitation, and case management
  • Responsible for maintaining active status as SUDP with DOH, including continuing education requirements

$32.56-35.10 hourly
