In this vital role, you will assist residents with daily activities, promote independence, and ensure a safe, nurturing environment. Your energetic approach will help foster positive relationships, enhance quality of life, and support the overall well-being of those in your care. This position offers an opportunity to make a meaningful difference in people's lives while working in a dynamic and rewarding setting. Essential Duties 1. Become familiar with the client's medical needs, medications, diagnoses, strengths and limitations, etc. 2. Monitor, support, and follow daily schedules and routines 3. Follow Individual Service Habilitation Plan goals and objectives 4. Discuss with the client, as necessary, issues regarding problem solving, information sharing, and crisis intervention 5. Assist client in maintaining a clean and orderly homeenvironment. 6. Assist client with independent living skills (i.e. budgeting, shopping, cooking, cleaning, hygiene, laundry, etc.) 7. Accompany the client into the community to engage in age/developmentally appropriate activities a. Develop a schedule/calendar of activities for the clients 8. Complete daily progress notes and service information in the client Communication Book 9. Maintain open line of communication with the team members (client, other staff, supervisor, and case manager) 10. Attend team meetings to provide input on organizational, behavioral, academic issues, etc. 11. Consult with Supervisor each week about any outstanding issues that need to be addressed 12. Follow the Positive Behavior Plans of clients who have a plan created and collect data as appropriate 13. Assist client with administering medication a. Ensure medication is in a lock box so that the client does not have access b. Ensure client is administered the appropriate dosage c. Staff during the 3p-11 p shift or live-in staf f will fill medication boxes each week 14. Complete time sheets accurately and submit/drop off time sheets on Sunday of each week or no later than 10:00 am on Monday of each pay period, biweekly 15. Complete Incident Reports once an incident occurs and submit to Supervisor immediately
Qualifications & Requirements:
To perform this job successfully, an individual must be able to perform each essential duty competently. The requirements listed below are representative of the minimum levels of knowledge, skill and/or ability required. Requirements may be modified to accommodate individuals with disabilities: Must have a valid Driver's License Must have reliable transportation Must have and maintain current liability auto insurance Must have a High School diploma or GED and/or 2 years' experience working with individuals with disabilities Must have good communication skills (oral and written) Must have adequate analytical and decision-making skills Must possess knowledge regarding the needs of persons with disabilities Must possess the ability to work with others and manage crises Must be reliable and dependable Must be trustworthy Must maintain confidentiality Must have good customer service skills Other Requirements Maintain 12 hours of training in "Best Practices" for working with individuals who are
