Job Overview We are seeking a compassionate and dedicated Direct Care Worker to join our team, providing essential support and care to individuals with diverse needs. In this role, you will be a vital part of creating a safe, respectful, and engaging environment for residents. Responsibilities Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor residents' health status by observing vital signs, patient monitoring, and reporting changes to healthcare professionals. Support medication administration following HIPAA guidelines and care plans established by medical teams. Provide companionship and emotional support to residents, fostering a caring and respectful environment. Prepare nutritious meals aligned with dietary needs and assist with meal preparation when necessary. Help with patient transfer methods using equipment such as Hoyer lifts or manual handling techniques to ensure safety. Maintain cleanliness of resident living areas through regular cleaning, laundry assistance, and general housekeeping duties. Support residents during outings or community activities, providing personal transportation assistance as needed. Follow all safety protocols including first aid, CPR certification requirements, and infection control procedures. Document care activities accurately in compliance with HIPAA regulations and facility policies. Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments. Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism. Familiarity with patient observation techniques, patient personal care assistance, and assisting with ADLs. Ability to operate patient transfer equipment like Hoyer lifts safely and effectively. Strong communication skills for client interaction and team collaboration. Experience working with individuals with disabilities or developmental delays supporting intellectual disabilities. Competence in basic medical tasks such as vital signs monitoring, medication administration, first aid, CPR, and hospice or geriatrics care. Ability to perform heavy lifting safely while maintaining manual handling best practices. Knowledge of home & community care practices including home health services and long-term care procedures.
