Job Description

Summary of Position The Certified Caregiver plays an integral role in the success of our team, our community and creating a place our residents are proud to call home. The primary purpose of this position is to provide residents with routine daily care and medication services in a caring and respectful manner in accordance with the assessment and service plan. Essential Job Functions Assist residents with activities of daily living (ADL) tasks, including but not limited to bathing, dressing, grooming, toileting, transferring, and mobility maintaining resident dignity and respect and in accordance with all policies, procedures, regulations and guidelines. Assist with moving and transferring residents into and out of bed, chairs, bathtubs, wheelchairs, lifts, etc. following all safety/lifting and transferring techniques. Communicate appropriate information (including incident reports information) and any observed or suspected resident change of condition to a supervisor immediately. Assist residents to walk with or without self-help devices as instructed. Encourage residents to socialize and participate in scheduled activities and outings and assist with those activities as needed. Respond promptly and courteously to all resident alerts, needs and/or requests for care, and provide assistance with care as needed. Observe emotional condition of each resident, promptly reporting all changes to the nurse. Follow and promote personal safety precautions and a safe environment for residents by observing infection control practices, following facility guidelines, and reporting unsafe situations to the nurse or management. Check each resident per service schedule to ensure that his/her personal care needs are being met in accordance with his/her wishes. Ensure resident safety by assisting in the performance of fire or other safety drills. Assist medications according to health care provider instructions following the Medication Administration Record and within the scope of your certification. Maintain accurate documentation of medication administration, medication effectiveness and any noted side effects, in accordance with established policy and procedures, standards of care and applicable state/federal regulations. Report any adverse reactions or side effects to supervisor or Director of Nursing. Perform other essential job-related duties as assigned. Knowledge and Critical Skills Be able to make independent decisions and follow instructions. Deal tactfully with personnel, prospects, residents, family members, visitors, government agencies/personnel, and the public. Capable of working with ill, disabled, elderly, and emotionally upset people within the facility. Communicate effectively in a manner that is sufficient for supervisors, team members, prospects, residents, and families. Maintain confidentiality in all aspects of the job to ensure resident privacy. Knowledge/proficiency of facility EHR. Education and Experience Must meet all applicable state and federal requirements for this position. Prefer 6 months or more of experience in a home care, assisted living, or long-term care facility. Supervisory Responsibility This position does not have any direct reports or supervisory requirements. Working Conditions and Physical Demands Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals. Physical Requirements - The Physical activities of this position involve: Ability to lift fifty (50) pounds; moving, lifting or transferring of residents may involve lifting up to 75 pounds. Ability to exert maximum muscle force to lift and carry to a height of 3 feet, push and pull to a distance of 5 feet. Ability to use abdominal and lower back muscles over time without fatigue. Ability to stand for extended periods of time. Ability to bend, stoop, stretch, twist, sit and reach. Fine motor skills. Cognitive Requirements - The Cognitive activities with or without prosthetics of this position are: Executes tasks independently. Ability to express yourself clearly and effectively verbally. Ability to concentrate on moderate details with frequent interruptions. Visual acuity. Hearing alertness. Environmental Requirements - An individual in this position may be exposed to: Ambient room temperatures and lighting.
