In this vital role, you will provide personalized support to promote independence, safety, and well-being in home and community settings. Your energetic approach and attention to detail will help create a nurturing environment for those in your care, ensuring they receive the highest quality of support every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by taking vital signs, observing symptoms, and reporting changes promptly Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary Administer medications accurately following care plans and maintain detailed documentation in compliance with HIPAA regulations Utilize specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces Support clients with memory care needs such as Alzheimer's or dementia by implementing specialized techniques and providing companionship Requirements Proven experience in home health, assisted living, nursing homes, or group home settings preferred Knowledge of patient observation, medication administration, and vital signs monitoring Certifications such as CPR, First Aid, and HIPAA compliance are highly desirable Ability to lift heavy objects safely and perform physically demanding tasks when necessary Experience working with individuals with developmental disabilities, autism, or dementia is a strong plus Excellent communication skills to collaborate effectively with clients, families, and healthcare teams Valid driver's license and reliable transportation may be required depending on client location Embark on a rewarding career where your dedication helps improve lives every day.
