• About the

Role:

• As a Home Health Caregiver, you will provide one-on-one care to our patients in their homes, focusing on their unique needs and goals. You will assist with daily living activities, such as bathing, dressing, and grooming, as well as provide companionship and emotional support. Your compassionate and empathetic approach will help patients feel comfortable, safe, and valued in their own homes.

• Responsibilities:

• Provide personal care assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting
• Assist with meal preparation and feeding, if necessary
• Provide companionship and emotional support to patients
• Assist with light housekeeping and laundry
• Monitor and report any changes in patient condition or behavior
• Develop and implement personalized care plans in collaboration with patients and healthcare teams
