Job Overview Join our compassionate team as an In-Home Caregiver, where you will play a vital role in providing personalized, high-quality care to individuals in their own homes. As an In-Home Caregiver, you will deliver essential services that enhance quality of life, foster trust, and create a positive environment for those you serve. Your dedication and expertise will help clients maintain dignity and achieve their personal care goals every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper manual handling and patient transfer methods such as Hoyer lifts when necessary. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with feeding as required, ensuring safety during meal times. Administer medications following prescribed care plans and medication administration protocols, maintaining detailed records of all medication-related activities. Support clients with cognitive impairments such as Alzheimer's disease or dementia through specialized memory care techniques and personalized engagement strategies. Provide transportation assistance for appointments, errands, or community outings to promote social interaction and independence. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and ensuring the home remains hazard-free. Document patient progress, communicate effectively with family members and healthcare teams, and update care plans regularly to reflect evolving needs. Experience Prior experience working with individuals with disabilities, neurodevelopmental disorders such as autism, or in senior care settings like assisted living or nursing homes is highly preferred. Demonstrated knowledge of patient observation techniques, patient monitoring, first aid, CPR certification, and emergency response procedures. Experience working with residents requiring hospice or palliative care is a plus. Familiarity with home health practices including home & community care experience and long-term care environments is advantageous. Ability to perform heavy lifting safely using proper manual handling techniques when assisting with transfers or mobility support. Strong communication skills for effective client interaction and coordination with healthcare professionals while maintaining HIPAA compliance. Knowledge of dementia care strategies, care plans development, medication management, and assisting with complex needs such as intellectual disabilities support or geriatrics is beneficial.
