Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote comfort and independence. Monitor patients' vital signs and observe for any changes in health status, promptly reporting concerns to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Support clients with mobility using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques, ensuring safety at all times. Administer medications accurately following care plans while maintaining compliance with HIPAA regulations to protect client confidentiality. Maintain detailed documentation of care provided, patient observations, medication administration, and any incidents or changes in condition. Provide transportation assistance for appointments or community outings, ensuring safe and timely arrival. Support clients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and engagement activities. Implement specialized care strategies for individuals with autism or developmental disabilities, fostering a supportive and respectful environment. Collaborate with healthcare teams to develop and follow personalized care plans that address each client's unique needs. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated knowledge of patient observation techniques, vital signs monitoring, medication administration, and basic first aid/CPR certification. Experience supporting individuals with neurodevelopmental disorders such as autism or intellectual disabilities supports success in this role. Familiarity with dementia care, Alzheimer's care, memory care strategies, and hospice or palliative care practices is advantageous. Proven ability to assist with ADLs using manual handling equipment like Hoyer lifts or patient transfer methods safely and effectively. Strong communication skills for engaging clients respectfully while maintaining confidentiality under HIPAA guidelines. Ability to perform light cleaning duties related to client spaces to ensure a safe and hygienic environment.
