HOME CARE CASE MANAGER

We are looking for a Licensed Vocational Nurse (LVN) to join our team as a Case Manager. This position is ideal for someone who enjoys both clinical coordination and operational oversight with our office staff, caregivers, and clients.

WORK SCHEDULE

This position will start as a 3 day a week position. If you do well in this role, the position may grow to 5 days a week. You will be required to work every other weekend plus a weekday shift. On the weeks where you are not working a weekend shift, you will work 3 weekday shifts. Shifts will be 8 AM to 5 PM in our office in Newport Beach. There is a possibility of the weekend shifts being worked from your home. Clinical Case Management Manage and oversee a caseload of home care patients Perform assessments and assist with care plan development Coordinate with physicians, families, and caregivers Monitor patient progress and ensure proper documentation from employees Provide training to caregivers in the field Driving to client homes and facilities Driving to pick up and drop off medication and other errands necessary for clients On-Call Support Handle client calls and urgent staffing questions from office staff Address client or caregiver concerns in real time Provide clinical guidance as needed Supervise open shifts and manage last-minute call-outs with the scheduling department Ensure proper caregiver-to-client matching Communicate schedule updates clearly and efficiently Qualifications Active California LVN license (required) Passing a LiveScan background check and registering as staff through the Home Care Services Bureau Have clinical experience working at a nursing home or hospital, etc. Home care or healthcare experience preferred Scheduling or staffing experience is a plus Strong organizational and multitasking skills Excellent communication and problem-solving abilities Comfortable working in a fast-paced environment What We Offer Hourly rate starting at $32/hour up to $35/hour Will provide a company phone and either a company laptop or iPad Mileage reimbursements per IRS regulations 401(k)
