Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance Prepare nutritious meals tailored to individual dietary needs, assist with feeding if required, and maintain cleanliness in dining areas Implement and follow personalized care plans designed by healthcare teams for residents with developmental disabilities, dementia, Alzheimer's, autism, or other special needs Maintain a clean and safe environment through regular cleaning duties and adherence to infection control protocols Document resident progress accurately in compliance with HIPAA standards and assist with patient monitoring during shifts Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home & community care environments Knowledge of patient care techniques including assisting with ADLs, medication administration, vital signs measurement, and patient observation Certifications such as CPR, First Aid, HIPAA compliance training preferred; willingness to obtain necessary certifications required for the role Strong communication skills to effectively collaborate with team members, residents, families, and healthcare providers Ability to perform heavy lifting safely using equipment like Hoyer lifts and handle physically demanding tasks with patience and professionalism Experience working with individuals with developmental disabilities, dementia (including Alzheimer's), autism spectrum disorder or in hospice care is highly desirable Attention to detail for documenting care activities accurately while maintaining confidentiality under HIPAA guidelines Join our team as a Direct Support Professional and become a vital part of empowering individuals to live fulfilling lives!
