Job Description

Job Summary Join our dedicated team as an In Home and Community Caregiver, where your compassionate support and hands-on assistance will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide in-home and community-based care services that promote independence, safety, and well-being. Your enthusiasm and commitment will help clients maintain their quality of life while receiving respectful, professional support tailored to their unique needs. This paid position offers a rewarding opportunity to foster positive relationships and deliver essential care within diverse community settings. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs, mental state, and overall condition; report changes to healthcare professionals promptly Administer medications following prescribed care plans while adhering to HIPAA regulations to ensure confidentiality Prepare nutritious meals aligned with dietary needs and assist with meal planning and cooking as required Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring safety during movement Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with healthcare standards Perform light housekeeping tasks like cleaning, laundry, and maintaining a safe environment to promote hygiene and comfort Skills Proven experience in home & community care or assisted living environments; group home or nursing home experience is highly valued Knowledge of patient observation techniques, medication administration, and vital signs monitoring Familiarity with specialized care such as Alzheimer's care, dementia management, memory care, autism support, and hospice care Ability to work effectively with individuals with developmental disabilities or other special needs; experience with residents' is a plus Strong understanding of HIPAA regulations to protect patient privacy and confidentiality Competence in using caregiving tools such as Hoyer lifts and performing heavy lifting safely when necessary Excellent communication skills for collaborating with healthcare teams, patients, families, and social workers Ability to perform basic cleaning tasks to maintain a hygienic environment; flexibility to handle emergency situations calmly and efficiently Join us in delivering compassionate, professional caregiving that enriches lives every day!
