In this vital role, you will assist clients with daily activities, monitor health and safety, and foster a warm, respectful environment. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote independence and dignity. Monitor patient health, medication reminders, observing changes in condition, and reporting findings accurately to healthcare teams. Driving clients to their doctor appointments, grocery shopping, errands. Prepare nutritious meals aligned with dietary requirements, assist with meal consumption, and maintain cleanliness in dining areas. Maintain detailed documentation of care provided, including patient observations, medication reminder records, and incident reports. Provide emotional support through active listening, engaging communication, and fostering a caring environment that respects residents' rights and individuality. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to perform patient observation and monitor vital signs accurately. Hands-on experience with dementia care, Alzheimer's care, memory care, hospice care or supporting individuals with neurodevelopmental or developmental disabilities is advantageous. Knowledge of HIPAA compliance and best practices for resident privacy and confidentiality. Proven skills in medication reminders, first aid certification, CPR certification, and safe manual handling techniques. Experience working with individuals with autism or intellectual disabilities supports your ability to provide tailored care in diverse settings. Familiarity with creating and following individualized care plans while respecting resident's rights and promoting independence.
