We are seeking a dedicated, client-focused Life Skills Trainer (LST) to join our team on a PRN basis with consistent weekly part-time hours . This role is perfect for someone who thrives on making a difference , enjoys working independently , and values strong professional relationships . You'll work directly with individuals in their homes and communities , helping them develop practical life skills that lead to lasting independence. What You'll Do As a Life Skills Trainer, you will: Provide one-on-one training and coaching to clients transitioning to independent living. Teach and support a wide range of essential life skills, including: ✅ Self-care and hygiene ✅ Communication and interpersonal skills ✅ Budgeting and money management ✅ Community mobility and transportation skills ✅ Time management, scheduling, and organization ✅ Goal setting and decision-making ✅ Home and household maintenance ✅ Emergency preparedness ✅ Advocacy and self-advocacy skills Accurately document session details and client progress in compliance with Medicaid Waiver Program requirements. Collaborate with clients and team members to ensure the highest level of client-centered care. About the Program Our Life Skills Training (LST) program serves Colorado's Medicaid population, supporting individuals as they transition from institutional settings to home and community-based living . The program's goal is to enhance physical, emotional, and financial independence , empowering clients to thrive within their communities. What We're Looking For We're not just looking for anyone; we're seeking someone who is: Independent and self-motivated, with strong time management skills. Client-centered and compassionate, with a genuine desire to empower others. Reliable and consistent; someone clients and colleagues can count on. Proficient in documentation and organization, with an eye for accuracy. Holds a bachelor's degree in human services, social work, psychology, or a related field (required). Experienced in working with individuals with disabilities or those transitioning to independent living (preferred). Able to successfully pass a background check and drug test. Physical Demands This position requires the ability to: Sit, stand, walk, and occasionally lift/push/pull up to 25 lbs. Communicate clearly and effectively with clients and team members.
