Direct Care Worker / Caregiver — Fort Collins, CO (80524) Join our compassionate team as a Direct Care Worker/Caregiver in Fort Collins, CO (80524), where your dedication will make a meaningful difference in the life of an individual requiring personalized support. This is a one-to-one assignment with a single client, offering up to 40 hours per week and a consistent, predictable schedule. Position Details Location:
Fort Collins, CO 80524 (client's private residence)
Hours:
Up to 40 hours per week with one client One-on-one care — no facility floor, no rotating assignments Duties Provide daily personal care: bathing, dressing, grooming, and hygiene. Provide bladder and bowel care, including incontinence care, with respect for the client's dignity and privacy. Assist with mobility, positioning, and transfers using safe body mechanics and the equipment specified in the care plan. Perform scheduled repositioning and daily skin care; watch for and promptly report any early signs of skin breakdown. Assist the client with eating. Provide medication assistance and support medical management tasks in accordance with the care plan and agency policy. Manage the client's medical equipment — set up, clean, monitor, and report any malfunction to the agency supervisor. Prepare meals daily according to the client's dietary needs.
Perform daily light housekeeping:
kitchen cleanup, bathroom cleanup, laundry, bed making, and trash removal, plus dishwashing, dusting, and floor care on scheduled days. Handle shopping and errands and support appointment coordination on scheduled days. Observe and report changes in the client's condition promptly to the agency supervisor. Clock in and out through the agency's electronic visit verification (EVV) system and document care accurately at every shift. Experience Hands-on experience providing total care to a physically dependent client — home care, group home, assisted living, skilled nursing, or hospital setting. Competency with transfers and safe patient handling, including mechanical lifts (Hoyer or sit-to-stand) and gait belts. Experience with incontinence care, catheter or bowel programs, and toileting support.
Understanding of skin integrity:
repositioning schedules, pressure injury prevention, and recognizing early breakdown. Comfort assisting with eating, including awareness of choking and aspiration risk. Familiarity with durable medical equipment such as wheelchairs, hospital beds, and lifts. Ability to follow a written care plan and to recognize, document, and report changes in condition. Knowledge of HIPAA and client confidentiality requirements. Reliable transportation and a valid driver's license.
