We serve individuals and families across through various Medicaid waiver programs, including C-HCBS, HCBS-EBD, IHSS, and others. Our mission is to help clients live safely, comfortably, and independently in their own homes with dignity and professionalism. Job Overview We are seeking a skilled and compassionate Caregiver to provide exceptional in-home support to our diverse client population. The ideal candidate will possess extensive experience in patient care, memory care, and assisted living environments. As a Skilled Caregiver, you will be responsible for delivering personalized care that promotes safety, independence, and well-being while adhering to all safety standards and care plans. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health status by observing vital signs such as blood pressure, pulse, and temperature; report any concerns promptly. Administer medications accurately according to prescribed care plans while maintaining compliance with HIPAA regulations. Support residents with mobility using patient transfer methods such as Hoyer lifts or manual handling techniques. Prepare nutritious meals tailored to dietary restrictions and preferences, ensuring proper food safety practices. Provide companionship and emotional support to clients with dementia or neurodevelopmental disorders, including Alzheimer's care and memory support. Assist with light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment. Utilize patient monitoring skills to identify changes in condition or behavior that require escalation or medical attention. Experience Candidates should have prior experience working in group homes, nursing homes, assisted living facilities, or home health settings. Experience working with individuals with disabilities—including developmental disabilities, intellectual disabilities, autism spectrum disorder—and those requiring specialized dementia or hospice care is highly valued. Familiarity with care equipment such as Hoyer lifts and knowledge of medication administration protocols are essential. Applicants must demonstrate proficiency in patient observation, documentation of care plans, CPR/First Aid certification, and manual handling techniques. Long-term caregiving experience supporting geriatrics or neurodevelopmental disorders will strengthen your application.
