Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and overall condition; report changes promptly Administer medications safely following care plans and HIPAA regulations to protect patient privacy Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking Use specialized equipment such as Hoyer lifts or transfer belts to safely move clients with mobility challenges Maintain a clean and safe environment through regular cleaning and organization of the home setting Support clients with cognitive impairments such as dementia or Alzheimer's by implementing memory care strategies and engaging in meaningful activities Follow detailed care plans developed in collaboration with healthcare professionals and social workers Document patient observations, care provided, and any incidents accurately in compliance with HIPAA standards Provide emotional support and companionship to promote mental well-being and social engagement Requirements Prior experience in home & community care, assisted living, nursing homes, or group homes is highly preferred Knowledge of patient observation techniques, medication administration, and vital signs monitoring Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus Experience working with individuals with developmental disabilities, autism, or dementia is advantageous Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary Strong communication skills to collaborate effectively with healthcare teams and family members Demonstrated compassion, patience, and professionalism in caregiving roles Valid driver's license and reliable transportation preferred for travel between client homes Join us in providing exceptional home health care that transforms lives!
