In this vital role, you will provide personalized, high-quality care to individuals in their homes, helping them maintain independence and improve their quality of life. Your energy and dedication will make a meaningful difference by assisting with daily activities, ensuring safety, and fostering a supportive environment. This position offers a rewarding opportunity to build trusting relationships while delivering essential care that promotes well-being and dignity. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by taking vital signs and observing for any changes in condition Prepare nutritious meals tailored to individual dietary needs and preferences Administer medications following care plans and HIPAA privacy regulations Support clients with memory care needs such as Alzheimer's or dementia through specialized techniques Maintain a clean and safe environment by performing light housekeeping tasks and assisting with cleaning routines Document care activities accurately, update care plans regularly, and communicate effectively with healthcare teams about patient progress Use equipment like Hoyer lifts or other assistive devices to ensure safe patient transfers and prevent injury Provide emotional support and companionship to enhance clients' mental well-being Follow all safety protocols including CPR and First Aid procedures to respond promptly in emergencies Skills Proven experience in home health, assisted living, or long-term care environments; group home experience is a plus Strong knowledge of patient observation, medication administration, and vital signs monitoring Familiarity with dementia, Alzheimer's, hospice, autism spectrum disorder, and developmental disabilities care techniques Ability to lift heavy objects safely and operate assistive devices such as Hoyer lifts or resident lifts Excellent communication skills for working with clients, families, and healthcare professionals Knowledge of HIPAA regulations to ensure confidentiality of patient information Compassionate attitude with a commitment to providing respectful, person-centered care Ability to work independently while following detailed care plans and protocols Join us in making a positive impact on the lives of those we serve!
