Duties Deliver personalized patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs and observe for changes in health status, documenting findings accurately to ensure proper care plans are followed. Administer medications safely according to prescribed instructions while maintaining compliance with HIPAA regulations to protect patient confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary, ensuring proper nutrition and hydration. Support clients with cognitive impairments such as Alzheimer's disease or dementia through specialized memory care techniques and patient observation to ensure safety and comfort. Assist with transportation to medical appointments, community activities, or errands using personal transportation assistance or mobility aids. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces in accordance with health standards. Collaborate with healthcare teams to develop and implement care plans tailored to each resident's needs, including those with developmental disabilities or neurodevelopmental disorders. Provide emotional support and effective communication to clients and their families, respecting residents' rights and promoting dignity at all times. Qualifications Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Knowledge of patient monitoring techniques, medication administration, first aid, CPR certification, and HIPAA compliance is required. Familiarity with specialized care such as Alzheimer's care, dementia support, hospice care, autism spectrum disorder (ASD), intellectual disabilities, or geriatrics is preferred. Ability to perform manual handling tasks including heavy lifting and patient transfers safely while adhering to safety protocols. Strong observational skills for patient safety and health status changes; experience in patient observation is essential. Excellent communication skills for engaging effectively with clients, families, and healthcare professionals. Ability to work flexible hours including live-in shifts; valid driver's license preferred for transportation duties.
