Job Description
Job Overview We are seeking a compassionate and dedicated Home Health Aide (HHA)/Caregiver to provide exceptional personal care and support to clients in their homes. The ideal candidate will possess a strong commitment to enhancing the quality of life for individuals with diverse needs, including seniors, individuals with disabilities, and those requiring specialized care such as dementia or hospice support. This role offers an opportunity to make a meaningful difference through attentive, respectful, and professional caregiving. The position is paid and requires adherence to all relevant home health care regulations and standards. Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility assistance. Monitor and observe patients' health status, documenting vital signs such as blood pressure, pulse, and temperature accurately within electronic health record (EHR) systems. Support medication management by administering prescribed medications following medical instructions while maintaining strict confidentiality in compliance with HIPAA regulations. Provide specialized care for clients with dementia or Alzheimer's disease, utilizing memory care techniques to ensure safety and comfort. Operate patient transfer methods safely using equipment such as Hoyer lifts or transfer belts, ensuring proper body mechanics and client safety. Perform light housekeeping tasks including cleaning, laundry, and meal preparation to promote a clean and healthy environment. Support end-of-life care with compassion, dignity, and respect for patients' wishes while coordinating with healthcare professionals as needed. Maintain thorough medical documentation of care provided, adhering to home health agency standards and regulations. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly desirable. Demonstrated proficiency in patient observation, medical terminology, catheter care, and vital signs monitoring. Experience working with individuals with developmental disabilities or geriatrics enhances the ability to deliver personalized care. Familiarity with EMR (Electronic Medical Records) systems and home health documentation practices is preferred. Knowledge of home & community care regulations along with hospice or heavy lifting experience is advantageous. Certification in first aid and CPR (Cardiopulmonary Resuscitation) is required; Basic Life Support (BLS) certification is a plus. Proven ability to communicate effectively with clients and healthcare teams while maintaining patient confidentiality at all times.
