Job Description
A Direct care Provider Supports individuals with disabilities, chronic illness, or other special needs in their daily living activities. The goal is to promote independence ,dignity, safety, and overall quality of life in a home and in the community - based setting. Assist Individuals with ADLs including bathing, dressing, grooming, toileting, and mobility. Provide companionship and emotional support Support with meal planning, preparation, and feeding as needed Assist with medication reminders (as permitted by state guidelines) Implement and follow individualized service plans and behavior support plans Provide life skills training , including budgeting, communication, and social interaction and safety skills. Ensure a safe and clean living environment Transport individuals to appointments, community outings, and activities (if applicable) Monitor and document changes in health, behavior, and progress Maintain accurate and timely case notes and service documentation Follow all state regulations, agency policies, and HIPAA confidentiality standards Report any incidents, abuse, neglect, or safety concerns immediately
