In this vital role, you will oversee program operations, support personal growth, and foster a safe, inclusive environment. Your leadership will inspire confidence and independence among those served, ensuring they receive comprehensive support tailored to their unique needs. Duties Develop and implement individualized plans that promote independence, focusing on daily living skills, community integration, and personal goals. Coordinate and supervise direct support staff to ensure high-quality care, including assisting with activities of daily living (ADLs), medication administration, and behavioral health needs. Conduct regular assessments of client progress through data collection, documentation, and reporting to monitor outcomes and adjust plans accordingly. Foster strong relationships with families, community agencies, and service providers to enhance support networks and resource access. Ensure compliance with all regulatory requirements related to home & community care services and maintain accurate records for audits and quality assurance. Drive transportation services by safely operating vehicles for client outings, appointments, or community activities as needed. Support clients in developing life skills such as cooking, cleaning, socialization, and employment readiness while promoting autonomy. Requirements Proven experience working in Independent Living environments. Experience in caregiving roles involving direct support or personal assistance in home or community environments. Valid driver's license with a clean driving record; ability to operate transportation vehicles safely. Knowledge of data collection techniques for tracking progress and reporting outcomes accurately.
