This position offers the opportunity to work closely with diverse populations, including seniors, individuals with developmental disabilities, and those recovering from illness. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and record vital signs, patient observations, and changes in health status to ensure ongoing safety and well-being Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking Administer medications accurately following care plans while maintaining strict compliance with HIPAA regulations to protect patient privacy Support clients with home & community care activities, including light cleaning, laundry, and maintaining a safe living environment Provide companionship and emotional support to reduce feelings of isolation and promote mental health Follow detailed care plans created by healthcare professionals and communicate any concerns or updates promptly Requirements Proven experience in home & community care or assisted living environments; group home or nursing home experience is highly valued Knowledge of patient observation techniques, medication administration, and vital signs monitoring. Certifications such as CPR, First Aid, HIPAA compliance training preferred; willingness to obtain necessary certifications if not already certified Ability to lift heavy objects safely using equipment like Hoyer lifts; physical stamina for active caregiving tasks Experience working with individuals with disabilities, autism spectrum disorder, or developmental disabilities is a plus Strong communication skills for effective interaction with clients, families, and healthcare teams Compassionate attitude combined with a proactive approach to safety and patient care Join us in delivering exceptional home health services that empower individuals to live comfortably and confidently in their own homes.
