The ideal candidate will be energetic, attentive, and committed to delivering personalized care that promotes independence, safety, and well-being. This paid position offers an opportunity to make a meaningful difference while working in a dynamic and rewarding environment. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient health by checking vital signs, observing symptoms, and reporting changes to healthcare professionals promptly. Administer medications accurately according to care plans while adhering to HIPAA regulations to ensure confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Maintain a clean and safe environment through regular cleaning tasks, laundry, and organization of personal spaces. Support clients in engaging with their community through social activities, appointments, and outings to promote social well-being. Follow detailed care plans designed by healthcare teams for clients with dementia, Alzheimer's, or other chronic conditions such as autism or disabilities. Skills Proven experience in home health, assisted living, nursing homes, or group home settings. Knowledge of patient observation techniques and ability to recognize signs requiring medical attention. Certification in CPR and First Aid; experience with medication administration preferred. Strong understanding of HIPAA compliance and confidentiality standards in healthcare environments. Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or wheelchairs. Excellent communication skills for working effectively with clients, families, and healthcare teams. Familiarity with dementia care, memory care strategies, hospice support, and working with individuals with developmental disabilities or autism.
