You are an experienced caregiver who values consistency, professionalism, and building a trusting relationship with one client. Each shift, you'll support a gentleman living in his own home after a recent hospital and rehab stay. A stroke has left him with significant right-sided weakness. He also has dementia and often thinks he can do more than he can safely do alone. He needs a calm, capable caregiver who protects his dignity, helps him make choices when he can, and stays ready when he needs hands-on help. Your presence helps set the tone for his day. Mobility support is a core part of this role. He uses an electric wheelchair, and transfers are done with a mechanical Hoyer lift. Company policy requires two people for the lift; a second person is available in the home to help you. He can become frustrated during transfers. Stay patient. Do not take it personally. Move slowly, explain what you are doing, and keep him safe. He has tipped in the wheelchair and can become uncomfortable. Help with positioning, including a pillow if he will allow it. The power chair is complex; he usually knows how to use it, but frustration can make him lose the sequence. Watch how he operates it so you can cue him without taking over. He likes space. Do not hover. A doorbell/call button is how he will reach you—keep the ringer where you can hear it and respond quickly. You assist with full personal care as needed: brief changes in bed, bed baths, dressing, grooming, toileting, and showering as tolerated. Reposition him regularly to prevent skin breakdown and keep him comfortable. He has a Foley catheter; you will assist with catheter care. He also uses compression boots twice a day for an hour. Remind him, encourage him, and help him get them on and off. He enjoys conversation and will ask questions about his care. Answer honestly, involve him in decisions when it is safe, and do not argue when dementia and safety collide. Prepare fresh, home-cooked meals. He is on a no-salt diet. Encourage hydration. Present food nicely when you can. Alcohol is not part of this home plan; mocktails are welcome. Light housekeeping is part of the role: laundry, dishes, trash, and keeping living areas neat and calm. When the day is quieter, stay useful. Sitting idle is not expected. There are two large dogs in the home. They are sweet and calm. There is also a cat he adores. Do not let the cat slip out the door. A caregiver binder is in the kitchen for daily notes and medication sign-off.
