This vital role involves delivering personalized support in a home setting, ensuring safety, comfort, and dignity for clients. The ideal candidate will possess a proactive attitude, excellent communication skills, and a strong commitment to enhancing the quality of life for those they serve. Duties Provide comprehensive personal care assistance, including help with bathing, dressing, grooming, and mobility support Monitor patient health through observation of vital signs and overall condition, promptly reporting any concerns Assist with Activities of Daily Living (ADLs) such as eating, toileting, transferring, and medication reminders Administer medications accurately following medical instructions and maintain detailed medical records using Electronic Health Record (EHR) systems Support clients with memory care needs, including dementia and Alzheimer's care, utilizing specialized techniques for safety and engagement Operate patient transfer methods safely, including Hoyer lifts and other mobility aids Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation Follow home healthcare regulations and ensure compliance with HIPAA standards to protect patient confidentiality Communicate effectively with clients, family members, healthcare providers, and team members to coordinate care plans Assist with end-of-life care when necessary, providing comfort and emotional support in hospice settings Requirements Proven experience working as a caregiver in assisted living facilities, nursing homes, or home healthcare settings Strong knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring Experience working with individuals with disabilities or developmental disabilities preferred Certified in First Aid and Basic Life Support (BLS), including CPR certification Familiarity with EMR/EHR systems for accurate documentation of patient information Ability to perform heavy lifting safely when required for patient transfers or mobility assistance Excellent patient observation skills combined with compassionate communication abilities Knowledge of home health care regulations and best practices for senior care and hospice services Ability to work independently in a live-in employment setting while maintaining professionalism and confidentiality Valid driver's license preferred but not required; reliable transportation is advantageous Join us in providing compassionate care that transforms lives!
