Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support patients with mobility and transfers using manual handling techniques such as Hoyer lifts and patient transfer methods to promote safety. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals tailored to dietary needs and assist with meal consumption to support overall health and well-being. Administer medications following prescribed care plans and ensure proper documentation of medication administration procedures. Provide transportation assistance for appointments or errands, ensuring safe and reliable client mobility within the community. Engage in meaningful activities that promote mental stimulation and social interaction, especially for individuals with dementia, Alzheimer's, autism, or developmental disabilities. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning and organizing living spaces regularly. Support hospice or palliative care needs by offering comfort measures and emotional support during difficult times. Document patient observations meticulously, including changes in behavior or health status, to inform ongoing care strategies. Qualifications Proven experience working with seniors or individuals with disabilities in settings such as group homes, nursing homes, assisted living facilities, or home health environments. Knowledge of dementia care, memory care techniques, Alzheimer's care, and neurodevelopmental disorders to provide specialized support. Familiarity with patient monitoring tools like vital signs measurement and CPR/First Aid certification to ensure safety during emergencies. Ability to perform heavy lifting and manual handling tasks safely while assisting with transfers using equipment like Hoyer lifts or patient transfer methods. Strong communication skills for effective client interaction and collaboration with healthcare teams while maintaining HIPAA compliance. Experience working with individuals with autism or intellectual disabilities supporting their unique needs through personalized care plans. Valid certifications in CPR and First Aid; additional training in hospice care or geriatrics is a plus. Demonstrated ability to handle multiple responsibilities efficiently within home & community care settings while maintaining compassion and professionalism.
