Job Summary Join our dedicated home care team as a Full-Time Home Care Floater / Personal Care Assistant (PCA) within a 25-mile radius! In this dynamic role, you will provide compassionate, personalized care to clients across various settings, including private homes, assisted living communities, and group homes. As a floater, your versatility and proactive approach will ensure seamless support wherever you're needed most. Duties Deliver direct patient support by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper patient transfer methods such as Hoyer lifts when necessary. Observe and monitor patients' health status by taking vital signs, documenting changes, and reporting concerns promptly to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking to promote healthy living. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and ensuring infection control protocols are followed. Provide transportation assistance for medical appointments or community activities using safe patient transfer techniques only when needed. Document all care activities accurately in compliance with HIPAA regulations and organizational policies while maintaining clear communication with team members. Skills Proven experience working in home health, hospice care, nursing homes, assisted living facilities, or group homes. Strong patient observation skills with the ability to recognize changes in condition or behavior. Knowledge of dementia care, Alzheimer's care, memory care, and caring for individuals with neurodevelopmental or intellectual disabilities. Familiarity with care plans, medication administration, patient monitoring, and resident's rights. Ability to perform manual handling tasks including heavy lifting and patient transfers safely. Excellent communication skills for effective client interaction and teamwork within diverse caregiving environments. Certifications such as CPR and First Aid are required; additional training in HIPAA compliance and HIPAA regulations preferred.
