Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by taking vital signs and observing for changes in condition or behavior Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary Use specialized equipment such as Hoyer lifts or resident's to safely transfer individuals with mobility challenges Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols Document care activities accurately, including patient observations, progress notes, and incident reports Requirements Proven experience in home & community care, assisted living, nursing homes, or group home settings preferred Knowledge of patient observation techniques, dementia care, Alzheimer's care, memory care, and hospice care practices Certifications such as First Aid and CPR are required; additional training in HIPAA compliance is highly valued Ability to work with individuals with developmental disabilities, autism spectrum disorder, or other special needs Physical capability for heavy lifting and long periods of standing or walking; comfort with patient monitoring equipment Strong communication skills to collaborate effectively with team members, patients, families, and healthcare professionals Demonstrated compassion, patience, and a proactive approach to caregiving Join us in providing exceptional personal support that empowers individuals to live healthier, happier lives.
