Duties Provide comprehensive personal care assistance, including bathing, grooming, dressing, and mobility support to clients with diverse needs. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, medication reminders, and transferring using patient transfer methods or equipment like Hoyer lifts. Monitor and observe patients' conditions by recording vital signs, recognizing changes in health status, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following medical instructions while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards. Support clients with specialized needs such as dementia care, Alzheimer's care, hospice support, or end-of-life care with patience and compassion. Maintain a clean and safe environment by assisting with light housekeeping tasks including cleaning, laundry, and meal preparation tailored to dietary requirements. Ensure proper documentation of medical records, medication administration logs, and patient interactions to uphold compliance and quality standards. Qualifications Previous experience working with geriatric patients or individuals with disabilities is highly preferred; assisted living or nursing home experience is a plus. Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) or CPR certification is required. Familiarity with home health agency operations, home & community care practices, and home health care assistance protocols is advantageous. Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts; physical stamina is essential. Excellent communication skills for client interaction and team collaboration; sensitivity to diverse cultural backgrounds is valued. Understanding of patient confidentiality laws including HIPAA regulations; experience maintaining medical documentation accurately. Experience working with individuals with developmental disabilities or dementia enhances your ability to provide personalized care in complex situations. $16.50 - $19.00 per hour
