Job Description
Overview Looking for a dedicated Caregiver (Supported Community Living Worker) and make a meaningful difference in the live of an. individual with developmental disabilities. In this vital role, you will provide compassionate support, promote independence, and foster a safe, engaging environment. Your energy and positive attitude will help empower personal goals and enhance quality of life. Responsibilities Assist individuals with activities of daily living (ADLs), including personal hygiene, grooming, meal preparation, and medication administration when required Engage in data collection to monitor progress, document care plans, and track or health changes accurately Provide transportation and driving support for appointments or community outings while ensuring safety protocols are followed Implement individualized care plans tailored to person's needs, preferences, and goals Collaborate with guardian, to coordinate care and support services effectively Qualifications Experience working with individuals with disablities.. or other disabilities in home or community settings Strong communication skills; ability to connect empathetically and clearly with individual Familiarity with medication administration procedures and safety protocols Ability to assist with ADLs and provide direct support in a respectful manner Valid driver's license and clean driving record for transportation duties
