In this vital role, you will provide comprehensive support to clients within their homes, fostering independence, safety, and well-being. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health status by observing vital signs and recognizing changes in condition, especially for clients with dementia, Alzheimer's, or neurodevelopmental disorders. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking. Support patients with mobility and transfers using proper techniques such as Hoyer lifts and patient transfer methods to ensure safety. Administer medications accurately following care plans while adhering to HIPAA regulations and privacy standards. Conduct patient observation and document progress or concerns to update care plans effectively. Provide transportation assistance for medical appointments, errands, or community activities, ensuring safe and reliable client mobility. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces. Support hospice or palliative care needs by offering comfort measures and emotional support during difficult times. Communicate clearly with clients' families and healthcare teams to coordinate care and ensure all needs are met compassionately. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care environments. Knowledge of patient monitoring techniques including vital signs measurement and dementia or Alzheimer's care strategies. Familiarity with assisted living facilities, nursing homes, or home health settings; experience working with residents' care plans is highly valued. Ability to perform manual handling tasks safely using appropriate equipment like Hoyer lifts; capable of heavy lifting when necessary. Strong understanding of HIPAA regulations to protect client confidentiality at all times. Excellent communication skills for effective client interaction and collaboration with healthcare providers. Certified in CPR and first aid; experience working with individuals with autism or neurodevelopmental disorders is a plus. Ability to perform light housekeeping duties while maintaining a respectful and caring attitude toward clients' personal spaces.
