The Opportunity
PurposeCare is seeking an experienced Community Care Program (CCP) Supervisor to join our Oswego Private Pay team.
This is a great opportunity for someone with home care and Illinois Community Care Program experience who enjoys being at the center of client care, caregiver support, and daily agency operations.
As the CCP Supervisor, you'll play an important role in ensuring our clients receive reliable, high-quality services while supporting the caregivers who make that care possible. We're looking for someone who is organized, responsive, comfortable solving problems, and able to build strong relationships with clients, families, caregivers, and internal team members.

What You'll Do
In this role, you will: Help oversee the day-to-day coordination and delivery of services for Community Care Program clients
Build strong relationships with clients, families, caregivers, and agency partners
Support caregivers and help address scheduling, service, attendance, and performance concerns
Help ensure clients receive consistent, dependable care according to their service needs
Respond quickly and professionally when client or caregiver issues arise
Partner with agency leadership to resolve concerns and maintain strong client satisfaction
Assist with caregiver communication, coaching, and ongoing support
Maintain accurate documentation and help ensure compliance with program and agency requirements
Identify opportunities to strengthen processes, caregiver retention, and the overall client experience
Serve as a dependable resource for both clients and caregivers while helping the Oswego team continue to grow What We're Looking For Previous experience within the Illinois Community Care Program (CCP) or an in-home care setting required
Bilingual Spanish speaker required
Strong understanding of home care operations and the needs of older adults and individuals with disabilities
Demonstrated ability to lead, support, and communicate effectively with caregivers
Strong organizational, problem-solving, and conflict-resolution skills
Ability to prioritize multiple needs and remain responsive in a fast-paced environment
Excellent verbal and written communication skills
A client-focused mindset with the ability to handle challenging situations professionally and compassionately
Ability to work independently while collaborating closely with agency leadership
Proficiency with Microsoft Office, including Word and Excel, preferred
Reliable transportation
High school diploma or equivalent required
