We are looking for someone with strong hands-on experience in home care, healthcare, client care coordination, caregiver supervision, CCP/Medicaid cases, and field operations . Position Overview As a Field Supervisor, you will help oversee client cases, support caregivers, conduct client visits, and assist with assessments for new clients. You will also help with office and field responsibilities as needed. This is a full-time position with 40 hours per week guaranteed . Service Area Our clients are located throughout Chicago and the Northern Suburbs . Services may take place in private homes, assisted living facilities, nursing facilities, hospitals, or other one-on-one care settings.

Responsibilities Responsibilities may include:
