Job Description

Overview We are seeking compassionate and dedicated Home Health Aides to join our team. In this role, you will provide essential support to individuals in their homes or community settings, assisting them with daily living activities and ensuring their comfort and well-being. Your contributions will make a significant difference in the lives of our clients, particularly those with developmental disabilities, Alzheimer's, or other memory-related conditions. Duties Prepares Aide notes for the clinical record. Reports any changes in the patient's condition to the Registered Nurse or Clinical Manager. Demonstrates competency in providing care/services and provides care/services in accordance with the Aide Care Plan. Maintains a safe environment when providing care/services for the patient. Adheres to the agency's policies and procedures. Maintains accurate and timely documentation of care/services provided. Performs the following activities only when instructed and supervised by a
Registered Nurse:
Assisting patients with bathing, shaving, mouth care, skin care, nail care (cleaning and filing only), and hair care. Straightening bed or changing bed linens. Checking patient temperature, pulse, respirations, blood pressure, and weight. Assisting with range of motion exercises and positioning. Assisting with rehabilitation program exercises as assigned by the Registered Nurse, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. Assisting with patient transfers and ambulation. Maintaining a safe environment and protecting patients from falls. Assisting patients with eating and meal preparation. Assisting patients with dressing. Assisting with elimination. Routine light housekeeping including dusting; dishes; keeping the kitchen and bathroom clean; cleaning the toilet, sink, and bathtub; emptying the trash; and laundry. Medication reminders. Attends and participates in staff meetings, educational in-services, and committee meetings, as appropriate. Participates in the agency's Quality Assessment and Performance Improvement (QAPI) program, as appropriate. Maintains confidentiality of all aspects of job performance. Practices proper infection control and utilizes PPE as indicated. Complies with all applicable federal, state, and local laws and regulations in performance of job functions. Performs other related functions as directed. Qualifications Previous experience in home health care, assisted living, nursing homes, or long-term care settings is preferred. Knowledge of medical terminology and experience with medical documentation is highly desirable. Certification in CPR is required; additional certifications in caregiving or home health aide training are a plus. Familiarity with catheter care and hospice care practices is beneficial. Strong observational skills to monitor patient conditions effectively. Ability to work independently while demonstrating compassion and patience towards clients. Experience working with individuals with disabilities or developmental challenges is an advantage. A commitment to maintaining a clean and organized environment for clients. Pay:
$18.00 - $19.00 per hour
