Job Description

Job Overview We are seeking a compassionate and dedicated Caregiver to provide exceptional support and assistance to our valued clients. This role is vital in enhancing the quality of life for individuals requiring personal care, medical support, and companionship. As a Caregiver, you will work closely with clients to promote independence, safety, and well-being in their homes or care facilities. Your energetic approach and attention to detail will make a meaningful difference in the lives of those you serve. This position offers an opportunity to develop your caregiving skills while making a positive impact every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or wheelchairs. Monitor and observe patients for changes in condition, documenting vital signs and health status accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications following prescribed schedules and medical documentation protocols, ensuring compliance with HIPAA regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and behavioral management. Support clients with medical procedures such as catheter care, wound dressing changes, and hospice or palliative care needs. Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary. Maintain a clean and safe environment by performing light housekeeping tasks like laundry, cleaning, and organizing personal spaces. Collaborate with healthcare teams to ensure comprehensive patient care, including updating medical records and communicating any concerns promptly. Skills Proven experience in patient observation and home & community care environments. Knowledge of medical terminology, medication administration, and vital signs monitoring. Familiarity with EMR/EHR systems for accurate documentation of patient information. Experience working with individuals with developmental disabilities or long-term care needs. Strong understanding of dementia care, Alzheimer's care, hospice support, and memory care strategies. Ability to assist with complex medical procedures such as catheter care and using assistive devices like Hoyer lifts. CPR certification and knowledge of safety protocols for patient handling and emergency response. Excellent communication skills to effectively interact with clients, families, and healthcare providers. Demonstrated ability to work independently while maintaining a positive attitude in fast-paced environments.
