Job Description

We are seeking compassionate and dedicated Direct Service Professionals (DSW / DSP) to become a part of our team at Louisiana Community Care! You will play a vital role in enhancing the quality of life for adults with developmental, physical, and mental disabilities in our supportive community home setting. Position Overview As a Direct Service Professional, you will provide hands-on support with daily living activities while fostering a safe, respectful, and nurturing home environment. This full-time role involves working closely with residents 30 years and older, promoting their dignity, choice, and community integration. Shifts may include days, evenings, weekends, and some overnights depending on program needs.
Key Responsibilities:
Assist residents with activities of daily living (ADLs) including personal hygiene, grooming, bathing, dressing, toileting, and mobility support Prepare nutritious meals, assist with feeding as needed, and maintain dietary requirements Perform light housekeeping, laundry, and home management tasks to ensure a clean and organized living space Support community outings, social activities, recreational programs, and skill-building opportunities to promote independence and inclusion Administer medications accurately (after proper training and certification) and monitor health conditions, reporting any changes to supervisory staff Complete detailed documentation including progress notes, incident reports, medication administration records (MARs), and daily logs in compliance with state regulations Encourage positive behavior support and help residents develop personal, social, and independent living skills Collaborate with RNs, LPNs, and the interdisciplinary team to implement individualized service plans Maintain a safe environment by following all safety protocols, infection control procedures, and emergency response guidelines
Qualifications:
High school diploma or GED required Previous experience working with individuals with disabilities, in caregiving, or in a similar residential setting is strongly preferred Must be at least 18 years old, have reliable transportation, and possess a valid Louisiana driver's license with a clean driving record Ability to pass a criminal background check, drug screening, and meet all Louisiana Department of Health DSW certification and training requirements Strong communication, patience, empathy, and problem-solving skills Physical ability to lift up to 50 lbs, assist with transfers, and remain on your feet for extended periods Flexibility to work varied shifts including weekends and holidays as needed Commitment to person-centered care and respect for individual rights and choices
