Essential Qualifications:
Develop and maintain comprehensive care plans tailored to the needs of clients. Coordinate services for individuals requiring senior care, home care, or support for people with disabilities. Provide behavior management strategies and support for clients dealing with dementia or other cognitive challenges. Assist with medication administration and ensure compliance with prescribed treatments. Conduct regular assessments of client progress and adjust care plans as necessary. Collaborate with healthcare providers, family members, and other stakeholders to ensure continuity of care. Facilitate communication between clients and their families regarding care options and progress. Maintain accurate documentation of client interactions, care plans, and any changes in condition.
