$25 per visit Job Summary The GAFC Direct Care Staff provides essential in‑home assistance to older adults and individuals with disabilities enrolled in the Group Adult Foster Care (GAFC) program. This role supports members in maintaining independence, safety, and stability within their homes through structured assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), as outlined in each member's Plan of Care. Key Duties ADL Assistance — bathing, grooming, dressing, toileting, feeding, ambulation, and transfers. IADL Support — meal preparation, light housekeeping, laundry, shopping, and appointment coordination. Provide medication reminders and assist members with prescription refill coordination. Complete Direct Care Aide visit logs accurately and submit monthly documentation to the supervising nurse. Observe and report changes in member condition, home environment, or safety concerns. Maintain confidentiality, professionalism, and compliance with GAFC program standards and agency policies. Qualification HHA, CNA, PCA, or equivalent caregiving experience preferred. Minimum 1 year experience working with elders or individuals with disabilities. Strong interpersonal communication and documentation skills. Ability to work independently in members' homes with professionalism and reliability. Valid driver's license and reliable transportation preferred.
